Let index start from 0

Least important proposal, so I left it for the end. This will allow user to start filenaming index from 0 instad of 1. Still 1 is default value, so user must explicitly choose 0 every time to use 0-based numbering.
Why? Why not :) It's easy to do.
There is no 0 page in books so I usually use 0 for book cover. So I start from 0 if I scan book cover or from 1 if I don't. Also I don't mind from weakening any constraints if possible.

That's a minor change, so it's up to you to decide if it's needed to be committed or better to stay in personal forks.
